AUTHORIZATION AND RELEASE FOR POST-OFFER MEDICAL EXAMINATION AND TESTING

By signing below, Applicant understands that his/her offer of employment is conditioned upon him/her passing a post-offer medical examination, including a drug screen for illegal, controlled substances, given by a physician and testing laboratory of the PIAD’s choice. Applicant understands that if he/she fails to appear for the medical examination and/or drug testing, or if he/she fails the drug test, his/her conditional offer of employment will be automatically be withdrawn.

Furthermore, Applicant understands that under certain circumstances, his/her failure to pass the medical examination may also result in the withdrawal of PIAD’s offer of employment.

By signing below, Applicant agrees to undergo medical and/or drug testing by a physician and/or facility chosen by PIAD. Applicant specifically authorizes PIAD’s physician and/or testing facility to reveal the results of any medical evaluation or test results to PIAD, and agrees to indemnify, to hold PIAD, the physician, and/or the testing facility harmless for the testing and the information provided to the Company.
